Scholarship Criteria
Students may apply who qualify with the below criteria:
1) Student who lives with Type I Diabetes
2) Students who have an immediate family member with Type I Diabetes
Applications and Awards:
Applications will be reviewed by the PEF Selection Committee Members and awards may be for partial scholarships of the annual tuition, books, and school fees.
Funds will be paid directly to the school by The Catholic Community Foundation who houses the PEF.
Applications will be accepted from September 1 ,2024-November 30,2024 and applicants will be notified if awarded a scholarship by February 15, 2025.
Scholarship Eligibility Requirements:
Applicant must have Type 1 diabetes or have an immediate family member who has Type 1 diabetes (immediate family defined as: mother, father, sister / stepsister or brother / stepbrother under age 18, living in the household at least 50% of the time)
Preference will be given to those individuals who themselves have Type 1 diabetes
A letter of endorsement from physician or Certified Diabetes Educator (CDE) that demonstrates that the applicant is proactively managing their diabetes and is actively involved in the diabetes community
Applicant will be a full time student in grades K-12th in one of the New Orleans Archdiocesan Catholic Schools
Only one student per household may apply
Applicant must show proof of acceptance into the school of choice
Only US citizens or permanent residents may apply

2023 Tax forms- Signed (please blind all social security numbers)
Essay from the student on their future educational goals and how the scholarship will enhance those for them
Endorsement letter from a medical professional
